RESUMEN

PURPOSE: Human papillomavirus (HPV) is the most commonly transmitted sexually transmitted infection. HPV infections have been on the rise among males, especially in the form of oropharyngeal cancer. Despite this, there is a gap in healthcare guidelines to increase HPV vaccine administration among males. In this study, we focus on the Indigenous population of North America and Oceania to determine existing barriers resulting in low HPV vaccination rates among the population. METHOD: We surveyed peer-reviewed literature on the awareness of HPV infection among Indigenous males in North America and Oceania. Using keywords HPV plus male, men or boy, and ethnical filters such as Indigenous, Aboriginal or First Nations, we retrieved 54 articles based on titles, of which 15 were included after reading the abstracts. RESULTS: Reported HPV awareness was generally low in Indigenous males in North America, with no peer-reviewed data from Oceania. The lower understanding by males compared to females was largely attributable to misconceptions about HPV-related diseases, their transmission, and prevention. Lack of awareness and concern toward the risk of contracting HPV infection in Indigenous males suggests an impediment in disseminating health information about this cancer-causing virus. CONCLUSION: Culturally sensitive education, with emphasis on Indigenous males, is needed to improve this group's HPV knowledge. Researchers should also engage meaningfully with Indigenous communities by building rapport to achieve a positive change in attitude.

Since the piezoelectric quality of bone was discovered in 1957, scientists have applied exogenous electrical stimulation for the purpose of healing. Despite the efforts made over the past 60 years, electronic bone growth stimulators are not in common clinical use. Reasons for this include high cost and lack of faith in the efficacy of bone growth stimulators on behalf of clinicians. The purpose of this narrative review is to examine the preclinical body of literature supporting electrical stimulation and its effect on bone properties and elucidate gaps in clinical translation with an emphasis on device specifications and mechanisms of action. When examining these studies, trends become apparent. In vitro and small animal studies are successful in inducing osteogenesis with all electrical stimulation modalities: direct current, pulsed electromagnetic field, and capacitive coupling. However, large animal studies are largely unsuccessful with the non-invasive modalities. This may be due to issues of scale and thickness of tissue planes with varying levels of resistivity, not present in small animal models. Additionally, it is difficult to draw conclusions from studies due to the varying units of stimulation strength and stimulation protocols and incomplete device specification reporting. To better understand the disconnect between the large and small animal model, the authors recommend increasing scientific rigor for these studies and reporting a novel minimum set of parameters depending on the stimulation modality.

BACKGROUND/OBJECTIVE: Perinatal mental health services are a current NHS priority and services are being increased for women. There is limited research on mothers' perspectives of these services and most research focuses on mother and baby units (MBUs). This study explored women's views of their experiences of generic wards, MBUs and crisis resolution teams. METHODS: A qualitative thematic analysis was conducted on written feedback on a service-user-designed questionnaire. One hundred and thirty-nine women recruited across 42 mental health trusts made comments. RESULTS: Two key themes were identified: support networks and staff authority. Support networks included subthemes relating to families, peers and staff. The theme of staff authority incorporated subthemes about communication, confidence in staff and service-user autonomy. All themes contributed to whether mothers felt safe in these services. Mothers reported the benefits of positive, non-coercive relationships with family and staff for their recovery. The findings highlight that the challenges women face in perinatal settings reflect the literature on general psychiatric services, particularly around coercion. CONCLUSIONS: Specific implications for mothers accessing perinatal mental health services: 1) integrated mental health care and support with babies; 2) support with separation from babies for mothers in acute wards; 3) improvement of women's relationships with social services across all services.

OBJECTIVES: Domestic violence and abuse (DVA) is highly prevalent, with severe adverse consequences to the health and well-being of survivors. There is a smaller evidence base on the health of DVA perpetrators and their engagement with healthcare services. This review examines the experiences of perpetrators of DVA of accessing healthcare services and the barriers and facilitators to their disclosure of abusive behaviours in these settings. DESIGN: A systematic review and meta-synthesis of qualitative studies. DATA SOURCES: A systematic search was conducted in Cochrane, MEDLINE, Embase, PsycINFO, HMIC, BNID, CINAHL, ASSIA, IBSS, SSCI (peer-reviewed literature) and NDLTD, OpenGrey and SCIE Online (grey literature). Each database was searched from its start date to 15 March 2020. Eligibility criteria required that studies used qualitative or mixed methods to report on the experiences of healthcare use by perpetrators of DVA. A meta-ethnographic method was used to analyse the extracted data. RESULTS: Of 30,663 papers identified, six studies (n=125 participants; 124 men, 1 woman) met the inclusion criteria. Barriers to disclosure of DVA to healthcare staff included perpetrators' negative emotions and attitudes towards their abusive behaviours; fear of consequences of disclosure; and lack of trust in healthcare services' ability to address DVA. Facilitators of disclosure of DVA and engagement with healthcare services were experiencing social consequences of abusive behaviours; feeling listened to by healthcare professionals; and offers of emotional and practical support for relationship problems by healthcare staff. CONCLUSIONS: DVA perpetration is a complex issue with multiple barriers to healthcare engagement and disclosure. However, healthcare services can create positive conditions for the engagement of individuals who perpetrate abuse. PROSPERO REGISTRATION NUMBER: CRD42017073818.

RESUMEN

It is essential that medical education should be equally representative of our society and subsequently the population that we will be serving as practicing physicians. As doctors, we have a duty of care to all members of our community. To align with these values, medical teaching should prepare future physicians to be able to treat patients from all backgrounds and ethnicities. After reflecting on medical education in the current global climate it is evident that there is unfortunately still a lack of ethnically diverse examples of clinical case presentations in medical education resources. This leads to students being ill-prepared in diagnosing certain illnesses in BAME patients which could result in a delay of treatment. It also causes BAME medical students to feel less included and acknowledged in the medical curriculum and can manifest in feelings of isolation and reduced self-worth.
